The lower part of the agitator has come off the splined shaft of the agitator shaft. The most likely problem is the lower half of the agitator is stripped.

1. Remove Agitator From Maytag Washer

This is more likely than the splines on the transmission are stripped.The upper part of the agitator snaps into the lower part and a screw, your extra screw, secures the upper half to the lower half.The image below should help with this problem.
